Patient screening questionnaires
We use Ocean, a secure patient communications portal, to send rating scales/questionnaires to patients assessing their mental health symptoms.
Patients can also access www.childmentalhealth.ca to complete necessary symptom questionnaires and rating scales prior to their appointment. No log-in required.
Click on: "Rating Scales" --> "Get Started" --> "I am Completing a Scale", search for "Joyce Health" as your "Healthcare Provider". Choose and complete the questionnaire(s) your doctor has asked you to complete.
- Usually, intake/first appointments will ask for the PSC-17 and Vanderbilt questionnaires.
- For followup appointments, your doctor will direct you on what questionnaires to complete prior to your appointment (e.g. SCARED, Columbia Depression Scale, PHQ9, etc)
